Salesforce Health Cloud – Elevating Patient Experiences

Below that is a Care Plan, customized and personalized specific to the patient’s needs. It also allows you to assign tasks to your patient. For instance, you can assign him a task to test his blood sugar levels 3 times a day. You can even login to the device data to check if he is actually doing that.

It allows you to Integrate third-party device data (like wearables) into the Health Cloud application to check for instance, if your patient’s sugar levels are increasing or a patient does not get proper sleep. In such cases, you understand that the patient is at risk and take the necessary means to get him back on course.

The Collaboration Tab in Salesforce Health Cloud allows you to get in touch with the patient’s primary care physician or any of the external or internal care team members. For example, you can see his Psychotherapist, Cardiologist, Diabetes Educator along with his primary Physician with whom you can communicate directly without leaving the console. Right from the Salesforce Health Cloud application, you can message him and let him know what you identified.
