Salesforce Health Cloud: Going Beyond Patient Records to Build Patient Relationships
Salesforce Health Cloud – In today’s digital era, Healthcare has changed a lot. And these changes are driven by three key factors – Consumer expectations, Technology, and Regulations. Consumer expectations are driving the market along with next-gen technologies including wearables, applications and medical devices revolutionizing the healthcare industry.
Though the current systems of records like RCM (Revenue Cycle Management) and EHRs (Electronic Health Record) have accelerated their benefits to go beyond just systems of record to provide subsidies, support from government and more, customers are constantly reminding us that it is not enough. They need systems to enable patient relationships that facilitate better communication, 1-to-1 relationship with their providers and can have frictionless patient experiences. And that is the goal and the vision in the new era of healthcare – Precision healthcare that drives better outcomes, higher customer satisfaction, and lower costs.
So how do we make Precision Healthcare a reality?
Presenting Salesforce Health Cloud – expressing a bold vision of delivering next-gen patient experiences beyond patient records. Quite simply, Salesforce Health Cloud is a patient engagement platform that will simplify the care coordination of your patients. Salesforce Health Cloud – Elevating Patient Experiences
Salesforce Health Cloud enables Healthcare providers to enhance patient experiences by Creating customizable and highly personalized connected experiences from patient acquisition through post-acute and home care. In a typical healthcare environment where a care coordinator manages hundreds of patients on a regular basis, the Salesforce Health Cloud gives instant access to all the patients’ data including those that have been assigned to you in the last 7 days. Your tasks are readily available and are prioritized by your patients. You will also see the messages that are related to your patients.
The Health Timeline on the top of the page of the Salesforce Health Cloud CRM (as in the image above) gives the complete story of the patient, allowing you to track both the clinical and non-clinical interactions. Below that is a Care Plan, customized and personalized specific to the patient’s needs. It also allows you to assign tasks to your patient. For instance, you can assign him a task to test his blood sugar levels 3 times a day. You can even login to the device data to check if he is actually doing that.
It allows you to Integrate third-party device data (like wearables) into the Health Cloud application to check for instance, if your patient’s sugar levels are increasing or a patient does not get proper sleep. In such cases, you understand that the patient is at risk and take the necessary means to get him back on course.

The Collaboration Tab in Salesforce Health Cloud allows you to get in touch with the patient’s primary care physician or any of the external or internal care team members. For example, you can see his Psychotherapist, Cardiologist, Diabetes Educator along with his primary Physician with whom you can communicate directly without leaving the console. Right from the Salesforce Health Cloud application, you can message him and let him know what you identified.
The traditional system means that you need to wait around for an answer given that Physicians are really busy and work for large healthcare systems. But with Salesforce Health Cloud, since the Physician has access through a Salesforce mobile application, he can be alerted that you sent a message to him asking for guidance; and in real-time no matter where he is, he can respond.
You can also Send him a survey – again without leaving the application. For instance, you can reach out to the patient to make a doctor’s appointment, and also fill out a survey in advance. This is how the Health Cloud allows you to seamlessly collaborate with the internal and the external team.
